General Information

Services are funded through the NSM Local Health Integration Network. Attendant Care Services involve both Personal Support and Homemaking services for individuals with significant physical disabilities or limitations living throughout Simcoe County.

Attendant Care Services for Adults with Physical Disabilities

TO BE ELIGIBLE for Attendant Care Services, individuals must:

  1. Be 16 years of age or older; be insured under the Health Insurance Act of Ontario;
  2. Have a permanent physical disability or limitations that requires physical assistance with activities; of daily living in order to accomplish such tasks safely and within a reasonable time;
  3. Be able to direct their own care;
  4. Be able to have medical and professional needs met by the existing community health network on a visitation basis;
  5. Be able to have their Attendant Care Services provided in their own home, also where required, in their competitive workplace or place where they are pursuing adult education courses to obtain a degree/certificate/diploma; (this is in Personal Support Services only).
  6. Must be able to live safely unattended.

Applicants are NOT ELIGIBLE for NSM LHIN funded Attendant Care Services if one of the following situations apply:

  1. The individual is receiving funding for Personal Support and Homemaking Services at a level that meets the individual's assessed plan of service; or
  2. The individual only requires assistance with homemaking activities; or
  3. Services are required on a short-term basis or from part of a rehabilitation program designed to assist an individual in recovering from an illness and/or accident; or
  4. The individual currently lives in accommodation and receives on-site Assistive Living Services that include Personal Support and Homemaking; or
  5. The individual is not capable of directing their own care; or
  6. The individual ceases to meet all of the eligibility requirements; or
  7. The person is currently receiving insurance payments or benefits for personal support and homemaking services as a result of a physical disability at the level sufficient for their assessed service requirements.

Assisted Living Services for Seniors

Assisted Living Services for seniors is a program designed to assist seniors to continue to live in their homes. This program offers a combination of personal support, homemaking services, security checks and care coordination.

You MAY BE ELIGIBLE for Assisted Living Services if:

  1. You have difficulty with personal care such as, toileting, bathing, dressing, or eating
  2. You are 65 years of age or older
  3. Demonstrate a need for daily personal support and are prepared to accept the assistance which cannot be met solely on pre-scheduled service
  4. Your family is having difficulty caring for you
  5. You have several ongoing medical concerns but are medically stable
  6. You are safe to remain at home between visits
  7. You have difficulty with tasks that keep you safe and well at home e.g. obtaining food, cooking, managing medications

INDIVIDUALS APPROPRIATE FOR SERVICES MUST RESIDE WITHIN THE GEOGRAPHIC AREA IN WHICH THE SERVICES ARE OFFERED. PRESENTLY OPERATING IN THE NORTH EAST END OF BARRIE, ONTARIO.

A FORMAL ASSESSMENT IS REQUIRED THROUGH THE NSM CCAC TO ESTABLISH ELIGIBILITY.

Provision of Service

Attendant Care Services involve both Personal Support services and Homemaking Services.

Personal Support:

  • Personal grooming and hygiene
  • Bathing and washing
  • Assistance with rising and retiring
  • Assistance with breathing
  • Toileting activities
  • Assistance with eating
  • Essential communication
  • Positioning and transferring
  • Assistance with mobility

Homemaking Assistance:

Only individuals who receive Personal Support are eligible for the Homemaking Assistance.

  • meal preparation
  • housekeeping
  • shopping and banking
  • care coordination ***
  • security checks or reassurance services ***
